Support the enrollment process for caregivers and patients in their homes, ensuring effective case management and compliance with state standards. Visit patients and caregivers in their homes to support the enrollment process to start of care.

Experience in field-based role. DCW certification. Excellent written and verbal communication skills. Reliable transportation and ability to travel as required. Self-starter personality and the ability to work independently with minimal supervision.
