LTSS Service Coordinator

The LTSS Service Coordinator is responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs, facilitating the Person Centered Planning process, and coordinating care with interdisciplinary teams.

Candidates must reside within 50 miles or 1-hour commute each way of a relevant Elevance Health location. This is a field-based role where the majority of time is working out in the field/community.
